HS Building Management prints photo IDs for faculty, staff, students, and visitors for the Health Sciences Building.
Following a request form submitted by a BAC, individuals may drop in at any time during business hours to obtain a photo ID. For groups of 3 or more, we kindly request that BACs make an appointment in advance.
Requirement
- Individuals in the building after-hours must have a UW-issued or affiliate-issued photo identification. A UW Husky Card may be used for this purpose.
- Photo identification (ID) badges are a supplement to Husky Cards.
- A Husky Card or HS Access Badge is needed to enter secure areas or the building after hours.
- A department may make a policy that their personnel must wear photo IDs at all times.

Policies
Individuals in the building after-hours must have a UW-issued or affiliate-issued photo identification. A UW Husky Card may be used for this purpose. Individuals are not required to display their photo identification, but must have it in their possession.
Departments with the following types of space may make a policy that their personnel must obtain and wear photo IDs at all times.
- Patient care areas.
- Animal quarters or animal laboratories.
- High security laboratories where sensitive research, hazardous chemicals, radioactive materials or select agents are present.
- Health Sciences honors photo IDs from affiliated institutions and University service departments (i.e., UWMC, HMC, Seattle Children’s, SCCA, Fred Hutch, Facilities Services, SLU, UW-IT).
- Visitors attending events/conferences occurring after-hours, on weekends or on holidays are not required to wear photo IDs while they are in transit to/from the conference/event.
- Persons traveling to and from the Health Sciences Library are not required to wear a photo ID.
- Any other after-hours visitor must be accompanied at all times by UW faculty, staff or student.
Departments will be charged $10.00 for all new and replacement HS photo ID badges.
Personnel Function
Photo IDs are color coded to designate personnel function as follows:
- Yellow: Faculty, staff, student employee
- Purple: Student
- Blue: Direct patient care
- Green: Essential personnel (determined by department)
- Red: Non-UW (i.e., contractors, consultants, vendors, volunteer)
- Photo identification badges are University of Washington property and must be returned to the issuing department upon termination of appointment, employment and/or registration.
- Each department is responsible for returning badges to HS Building Management when faculty/staff/students terminate.
